Every denied claim costs twice: once in revenue that arrives late or not at all, and again in the staff time it takes to fix and resubmit. Many denials come from avoidable issues such as wrong patient details, missing authorisations or simple coding slips. That makes them one of the best places to recover money in a practice.

These are the habits we see in claims teams that keep denials low.

1. Verify eligibility before the visit

Check coverage, plan details and patient information before the appointment, not after. Confirm the insurance ID, the policy holder and whether the plan is active. Many registration and eligibility denials disappear with this one step.

2. Track prior authorisations closely

Know which services need prior authorisation for each payer, request it early and record the approval number. Missing or expired authorisations are a common and very preventable cause of denials.

3. Scrub every claim before it goes out

Review claims for missing fields, mismatched codes and modifier errors before submission. A short checklist, or the claim scrubber in your practice management system, catches most problems while they are still cheap to fix.

4. Respect timely filing limits

Every payer sets a deadline for submitting claims and appeals. Keep a simple list of filing limits by payer and make sure no claim sits in a queue long enough to miss them.

5. Work denials fast

A denial is easiest to fix while the visit is fresh and the deadline is far away. Aim to review every denial within a few days of receiving it, correct what can be corrected and resubmit or appeal promptly.

6. Track denials by reason

Group denials by reason code and payer each month. Patterns show you where the process breaks: maybe one location skips eligibility checks, or one payer keeps rejecting a particular code. Fix the cause and those denials stop coming back.

7. Follow up on silence

Claims that are neither paid nor denied are easy to forget. Review unpaid claims by age every week and call or check the payer portal on anything that has gone quiet.

Where a dedicated team helps

A dedicated claims team can do this work every day without being pulled away to answer phones or check in patients. Teams in Sri Lanka can work your queue overnight, so eligibility checks are done before your first appointment and denials are reviewed before your staff arrive.

  • Eligibility and benefits checks for upcoming appointments.
  • Claim preparation, scrubbing and submission.
  • Denial review, correction, resubmission and appeals.
  • Aged claims follow-up with payers.
  • Monthly reporting on denial reasons and trends.
